What we collect and why
We collect your name, email, selected services, and any phone number, referral information, or comments you choose to provide. Jonathan Lu uses this information to respond to you and maintain inquiry records. Please do not submit medical records, payment information, or other sensitive details through this form.
Where your information goes
Submissions are stored in a private Supabase database and emailed to jon@hardhatwellness.com using Resend. Vercel hosts the website and processes form requests. Cloudflare Turnstile checks for automated abuse and may process technical information such as your IP address and browser information. These service providers process information to operate the form. Inquiry records are not available to website visitors, and the form does not add you to a marketing list.
Access and record-keeping
Inquiry access is intended for Jonathan Lu through his private service accounts and email inbox. Records are retained for follow-up and record-keeping until they are deleted. Copies in email, exports, and provider backups may have separate retention periods. This form is for initial inquiries, not clinical records.
